Printed Power of Attorney forms
The Office of the Public Guardian and Trustee (OPGT) no longer provides printed Power of Attorney (POA) forms. Instead, you can access the free POA form online in a redesigned, fillable format that is easier to complete or alternatively, a PDF designed to be printed for manual completion.
This change supports the government’s initiative to improve efficiency and ensure public access to essential forms, while saving time and promoting usability.
If you don’t have access to a computer or printer, you may access and print the form using public computer terminals, such as those at local libraries.
